As a new employee, there are a few documents that you will need to complete before you meet with your Human Resources recruiter at your sign-on appointment. The information below is intended to help you prepare for your sign-on meeting at the City of Burnaby and to assist you with questions you may have so that your start at the City flows smoothly and seamlessly.

Preparing for your Sign-On Appointment

At your sign-on appointment, you will meet with a member of the Human Resources Recruitment team to go over your paperwork and get you onboarded to start your new position with the city. At this time you will need to submit your filled out, hard copy forms (linked below) to the recruiter. If you have have any questions about completing these forms, your recruiter will be able to assist you during your sign-on meeting. 

Please note that if you don't have access to a printer, please complete these forms electronically and email them to your recruiter for printing prior to your sign-on.

What to Bring to your Sign-On Appointment 

Please bring your Driver's license or Government-issued Photo Identification, Social Insurance Card or letter of assessment from your tax return, void cheque to set up direct deposit, and Care Card for you and any dependents. Additionally, all City of Burnaby employees reqire a photo identification card. At sign-on we will take a quick photo of you and your employee ID card will be issued when you start work.
