Preparing for your Sign-On Appointment

As a new employee, there are a few documents that you will need to complete before your first day on the job!

At your sign-on appointment, you will meet with a member of the Human Resources Recruitment team to go over your paperwork and submit your completed forms (linked below) to the recruiter. If you have any questions about completing these forms, your recruiter will be able to assist you during your sign-on meeting.

Please note that if you do not have access to a printer, please complete these forms electronically and email them to your recruiter for printing prior to your sign-on appointment.

Regular & Temporary Full Time Employees

Auxiliary & Regular Part Time Employees

What to Bring to your Sign-On Appointment 

  • Driver's License or Government-issued Photo Identification
  • Social Insurance Card or Letter of Assessment from your tax return
  • Void Cheque to set up direct deposit
    Note: If you don't have a void cheque, ask your bank to provide you with a direct deposit form
  •  Care Card for you and any dependents (Regular Full Time, Temporary Full Time and Regular Part Time appointments only)



All City of Burnaby employees require a photo identification card. At sign-on we will take a quick photo of you and your employee ID card will be issued when you start work.
